The dimensions are usually nominal, meaning the actual size is slightly smaller due to planing and smoothing.
The sizes refer to the thickness:
4/4 (four-quarter) is approximately 1 inch (2.54 cm) thick
8/4 (eight-quarter) is approximately 2 inches (5.08 cm) thick
The "quarter" system is a traditional way to measure lumber thickness, where 1/4 represents 1/4 inch of thickness.
